FINRA fined $20,000 and suspended Melissa Ann Strouse (CRD #3200452) located in Cave Creek, Arizona for failing to timely update Uniform Application for Securities Industry Registration or Transfer (Form U4) forms for representatives of her member firm to disclose outstanding liens, judgments, bankruptcies, and compromises with creditors, despite having notice of those events. The findings stated that as the chief compliance officer (CCO), Strouse had the responsibility to amend the representative’s Form U4s within 30 days of discovery of an event requiring an amendment. The suspension is in effect from September 16, 2019, through January 15, 2020.
